Dr. Patel oversees all operations since founding VHL in 2016. He is a working physician, trained at the University of Kansas Hospital, and holds an MBA from Indiana University's Kelley School of Business. He has served as Vice President of Ambulatory and Population Health for a large health system in Minneapolis, as well as system Chief Medical Officer.

Prabhakar directs complex software design and peripheral device integration. He brings expertice in electronic medical record systems, accounting management software, smart mail software, complex data analysis and security management systems. Prabhakar was been awarded a Master’s Degree in Computer Applications and is a certified programmer of .Net Technologies.

Sakowicz guides VHL Care Coordinators to ensure successful patient interactions. In her role as a Care Coordinator, she directly facilitates patient wellness. She has been a Registered Medical Assistant for 19 years with experience in family practice medicine, internal medicine, oncology, cardiology, and chiropractic care.

Pam Zimmerman, an expert in social work and aging services, is the Director of Care Coordination and Client Relations for Vital Health Links. Prior to joining Vital Health Links, Pam worked for the Metropolitan Area Agency on Aging as the Director of Information and Assistance where she managed the Minnesota Senior LinkAge Line, an integrated service system designed to connect older adults and family caregivers to resource and benefit options. Pam holds a B.A. in Social Work and an M.A. in Gerontology. She is a Licensed Social Worker and is certified as a Care Transitions coach and a professional gerontologist. Pam also serves as a board member of the Minnesota Gerontological Society. “In my 30-year social work practice, I have seen the effects of our fragmented health and social systems on those seeking services. Vital Health Links is reimagining health care to truly integrate these systems and promote quality of care, quality of life, and vital aging for older Americans. I am honored to be a member of the Vital Health Links team, working alongside patients, family members, health care and community-based providers to promote person-centered coordinated care, patient engagement, and healthy aging.”
Ajeet has worked in the software development business for the past 9 years. His expertise lies with complex software design and integration of peripheral device integration. Currently, Ajeet manages a large team of highly skilled Engineers in his role as Vice President of Hard Shell Technologies. He is also responsible for customer acquisition and project management. Ajeet has obtained a Master’s degree in Computer Applications, and is a certified programmer of .Net Technologies. Through the course of his career, Ajeet has developed numerous electronic medical record systems, accounting management software, smart mail software, complex data analysis and security management systems.

During her time in graduate school, she completed a communications externship with the Mayo Clinic Department for Scientific Communications, and before joining Vital Health Links served as Internship Liaison for Girls Inc. at the YWCA of Minneapolis, coordinating communication and internship opportunities with community partners. JoJo believes that empowering individuals, families, and communities begins with access. “There is a wealth of health data and tools available to both providers and the public—Vital Health Links is making these tools accessible and adoptable to improve quality of care, patient satisfaction, and health care expenditure.”
